POSITION OVERVIEW:
Under supervision of the Program Director, the Behavioral Health (BH) Specialist provides patients advocacy, outreach, education, and care management services. Skills and competencies required for the position include communication, cultural competence, training professional experiences, and education. The activities are tailored to meet the unique needs of the communities. This position includes no managerial or supervisory responsibilities.
KEY ESSENTIAL FUNCTIONS:
- Provide crisis intervention on as needed basis, either via telephone or in-person
- Participate in grant related workshops and trainings
- Participate in daily IMT staff and organizational meetings
- Practice according to the principles of recovery oriented and trauma informed service delivery
- Offer and deliver brief structured counseling interventions with step-by-step guidance from provider facing apps specific to the client’s symptom severity and areas of need.
- Communicates effectively with client, collateral contacts, outside agencies, referring agencies, and/or outside providers as needed for continuity of care.
- Work with the Program Director and Team Leaders to develop and implement a strategic and targeted outreach plan for the program.
- Conduct outreach in identifying and locating potential clients
- Complete all documentation timely i.e., progress notes, comprehensive assessments, POC, consents, etc., in accordance with departmental policies.
- Participate in quality improvement activities, projects, and reviews.
- Attend Supervision with supervisor as scheduled and be prepared to discuss topics around caseload, engagement, work related concerns, barriers, trainings, etc.
- Screen prospective clients in the field and forward screening documentation to clinical supervisor for review.
- Coordinate with other internal and external staff and programs so that opportunities for participants can be maximized.
- Promote linkage development and monitor effectiveness of linkages with other service providers via phone, face to face meetings and formal case conferences.
- Use appropriate technology tools to accomplish job functions, understand and utilize available technology as customer service, communication, and data gathering tools.
- Maintain files and records of outreach activities conducted and other general reporting as assigned.
- Complete all documentation timely i.e., progress notes, comprehensive assessments, POC, consents, etc., in accordance with the BAHN, Health Homes and departmental policies.
- Provide direct coaching, education, and advocacy in linking, engaging and retaining clients in services identified in the Plan of Care.
- Performs related duties as assigned by supervisor.
REQUIREMENT:
- Bachelor’s degree in social work, psychology, mental health counseling, criminal justice, or other health and human services related field required.
- CASAC credentialed preferred.
- Two (2) years’ experience in successful community outreach/recruitment or volunteer coordination with expertise serving those with mental health conditions, substance use disorder, and/or histories of homelessness.
- Bilingual (English/Spanish); ability to communicate effectively orally and in writing.
- Excellent time management, organization, motivation, team-oriented, communication, and computer skills, including Microsoft Windows, Excel, PowerPoint, and the internet.
