The Community Outreach Worker will be responsible to engage our communities in need. The Community Outreach Worker will also be responsible for risk assessment during intake process in order to determine prevention needs and enrollment in the appropriate program services. The Community Outreach Worker will also assist with appointment scheduling, transportation, health insurance forms and other social services reinforces HIV/HCV/STD medical education with the patients and provides support to the patient throughout the process.
This position pays $24.03 per hour.
KEY ESSENTIAL FUNCTIONS:
- Outreach persons with HIV who are not yet linked to services, through conducting street, venue and agency-based outreach, conduct awareness raising activities, including tabling, health fairs, etc. to increase awareness of HIV testing, linkage to care and other specialty services.
- As an outreach worker, build caseloads, find clients lost to contact, and contact clients to help keep them engaged in care and services.
- Clarify information and answer questions regarding disease pathology, treatment eligibility and evaluation, HIV/HCV/STD treatment and medications, and managing co-morbidities
- Responsible for increasing the enrollment, engagement, and retention in care of HIV population
- Support patients in retention in care and medication adherence
- Provide and/or arranges transportation for clients in an as-needed basis.
- Respond to phone calls, written, and other forms of inquiries within required timeframe.
- Link patients to community resources to mitigate barriers to care
- Attend and assist in coordinating support group
- Update clinician on all new developments concerning patient
REQUIREMENTS:
- High School Diploma required.
- Must be vaccinated against covid-19
- Associates required/bachelor’s degree preferred.
- Three (3) year experience navigating systems for individuals with HIV
