Job Description
Under the supervision of the Assistant VP and Program Director, the Geriatric Social Worker on going case management services to low-income members and families and including assessment, counseling, referral, advice and resource identifications; helps families identify affordable permanent housing; assist with transportation act as liaison with other services agencies to identify assistance; maintains, completes, updates client files. This position is characterized by the high degree of confidentiality, independent action and on the delivery of non-judgmental assistance while performing day-to-day duties.
The salary is $50,000/Year- Bilingual Spanish preferred
KEY ESSENTIAL FUNCTIONS:
- Providing services to elderly clients and patients (over the age of 60) with mental illnesses, emotional distress, and psychological disorders.
- Assessing the needs, strengths, weaknesses, situations, and support systems of elderly clients and patients, in an effort to develop short-term and long-term goals and treatment plans.
- Developing treatment plans that are designed to improve the mental health and well-being of elderly clients and patients.
- Helping clients and patients adjust to life changes (i.e. illnesses, divorces, debt, death, and/or unemployment).
- Researching and referring elderly clients and patients to resources (i.e. government housing, medical services, Meals-on-Wheels, transportation, and food stamps).
- Helping elderly clients and patients sign up for government assistance (i.e. Medicare)
- Tracking the progress of elderly clients and patients.
- Evaluating geriatric mental health services to ensure that they are still effective.
- Acting as a liaison and advocate for elderly clients and patients.
- Providing assistance and support to the families of clients and patients.
- Arranging social activities and gatherings for elderly clients and patients.
- Provide monthly Quality Assurance Metrics each month for the Board Report (should be in PowerPoint and Graphs that are easily read).
- Must create activities that are complementary to mental health wellbeing and stress reduction. Must work with Program Director to ensure these workshops/activities are scheduled weekly.
- Enter all Progress Notes into PeerPlace or any other electronic record as mandated by the state of NY and Acacia Network. Notes must be entered daily.
- Other duties as assigned by the Program Director and Assistant Vice President.
- Must be willing to travel to various sites across the NYC area.
- Must participate in continuing education.
**EDUCATION AND EXPERIENCE REQUIRED: **
- Must be fully vaccinated against covid-19
- Bachelor's degree in social services or another related human services field.
- Minimum of two (2) years' experience working with under privileged and/or older adults.
- Bilingual English/Spanish preferred
- Must have a moderate level of computer skills specific to Microsoft Office Suite.
