POSITION OVERVIEW:                                                                                                                        

Under the direction of MAT Program Director, the MAT Program Manager will oversee the management of Primary Care’s MAT Program and will ensure the provision of monitoring the program’s success. The Manager will work out of multiple sites across New York City and work closely with the sites’ clinical leads to oversee the administrative aspects of the program, grants and projects, and lead the integration efforts.  Part of the responsibilities includes direct oversight of non-licensed MAT staff, ensure patients gain access to, and maximize the benefits of needed medical, social, educational and other support services though advocacy, outreach and education. The MAT Program Manager will also work to pursue new grant funding opportunities and ensure the success of project implementation, monitoring, reporting, budget expenditures, communication, and coordination. 

The MAT Program Manager must have knowledge of community resources and counseling/social work practices with high-risk populations. This individual must have experience working with persons in crisis.  Must be able to work in a variety of settings with culturally­ diverse families while having the ability to be culturally sensitive and appropriate. The goal of the MAT Program Manager is to supervise, monitor, train and implement all staff for optimal operations of the program.         

The annual salary for this position is $80,000.

KEY ESSENTIAL FUNCTIONS:                                      

    Recruit, supervise, manage, develop and evaluate performance of all program staff on an ongoing basis in accordance with applicable performance standards (with the exception of the clinical staff).  

    Collaborate with the HR team to formalize a development plan for staff as well as manage employee relations issues such as provide employees with formal write-ups and suggestions for improvement, and administer terminations as needed.

    Under the direction of the Director, lead the MAT Program, team, services and activities for members with co-occurring mental, medical and behavioral health conditions.

    Responsible for all reporting for HRSA (include annual UDS report) PHS, OASAS, DOH and internal reporting monthly and as required. This includes ensuring the integrity of the data and timely submissions. 

    Conducts and participates in internal and external meetings, conferences and seminars as a representative of Primary Care which may include Board of Directors meetings held afterhours and on weekends. 

    Review epidemiological and surveillance data to develop needs assessments and tailor the outreach and engagement strategy to high-risk communities.   

    Solicit, prepare and submit grants and budgets for new funding opportunities to bolster these programs and vulnerable populations.  

    Development of policies and procedures for MAT Program

    Ensure MAT staff and care coordination staff are trained in telemedicine; ER tasking alerts processes to ensure compliance including comprehensive addiction treatment services and MAT

    Ensure Program staff coordinate exchange of information between behavioral and medical health providers to develop comprehensive care plans 

    Develop quality improvement tracking tools, initiate QI initiatives, and run PDSA to ensure program goals and deliverables are met. 

    Monitor data to address quality measures, trends or potential improvement opportunities including provider issues, care gaps and patient needs

    Facilitate Case Conferencing for complex cases

    Develop and oversee system to ensure Program staff assess patients’ stages of treatment recovery and collaborate with team for appropriate linkage of services (methadone, chemical dependency outpatient program, detox, rehab, harm reduction groups)

    Facilitate MOU's/ Linkages with other harm reduction CBO's

    Supports and monitors Project Implementation Plans to assure appropriate project direction and progress

    Implement tracking system and ensure documentation of all direct and indirect services provided

    Engage in outreach activities to include visits to healthcare facilities within the network and develop positive relationships, encourage referrals and integration, including mobile van outreach

    Strengthen community partnerships to better leverage SUD and mental health community resources and support

    Ensure Program staff provide training and education to patients, families and communities focusing on SUD prevention and treatment, mental health, stigma, neo-natal abstinence syndrome, trauma informed care, suicide prevention, pain management and opioid overdose.

    Collects baseline and performance metrics by provider site. Establishes and documents project specific performance on project implementation plans. Measures performance against qua1terly improvement targets in preparation for quarterly reports to the CSO and the State

    Performs other duties as assigned


REQUIREMENTS:                                            

    Masters preferred, Bachelor's degree required

    1-3 years' experience working with members in a medical environment

    1-3 years supervision experience

    Behavioral/Substance Use experience preferred

    Must have excellent verbal and written communication skills

    Expert at using full suite of MS office products

    Interpersonal/human relations skills

    Organizational skill

    Ability to maintain confidentiality
