Position Overview

Under supervision of the Senior Administrator of Population Health (SAPH) , the Quality Improvement (QI) Coordinator is responsible for both assisting and leading quality improvement initiatives of all service areas including medical, behavioral, specialty and operational. This position will drive initiatives within population health management and continuity of care, efficient use of electronic health record (EHR), Patient Centered Medical Home (PCMH) requirements, Uniform Data System reporting, and other clinical quality related activities. 

The QI Coordinator will provide support to interdisciplinary primary care teams at each site in the interpretation/understanding of quality and performance improvement activities, engaging staff and providers from the inception to the development, implementation and training of compliance and performance improvement activities and collaborate with other key stakeholder to maximize reimbursement relating to quality and value-based contracts.

This position will use clinical applications software to design interventions support teams in carrying out the interventions, and in capturing data to support workflows and measure improvement in pursuit of clinical excellence. This work is carried out in support of the mission and goals of the La Casa De Salud. 

Key Essential Functions

  • Ability to conduct Quality Improvement projects that takes into considerations research of best practices, data analysis, implementation of PDSA, root cause analysis, lean management, and other QI strategies.
  • Ability to present and engage multi-disciplinary stakeholders and lead workgroups to improve processes and clinical outcomes.
  • Support grant funded programs in evaluating deliverables and quality improvement efforts. 
  • Act as Team Leader/Facilitator for the Quality Coordinator and PC improvement teams. 
  • Support Primary Care efforts to enhance clinical outcomes and maximize revenue potential with the MCOs’ Quality Incentive Programs and Value Based Contracts. 
  • Conduct chart reviews/audits for care gaps and workflow compliance. 
  • Analyze care gap data, develop outreach strategies, monitor efforts, and review trends, areas of opportunities and barriers.
  • Assist in internally publishing best practices, clinical workflows, and educational material to improve clinical measure outcomes.
  • Development of dashboards to support QI projects and various Primary Care needs
  • Maintain a working knowledge of clinical measure regulatory standards such as HEDIS and NQF and any other measure steward guidelines, as applicable
  • Facilitate patient satisfaction survey and cycle time collection and analysis.
  • Facilitate the provider peer reviews, collection and analysis.
  • Prepare reports and presentation for all levels, including Board level reports. 
  • Maintain and update the QI intranet SharePoint as needed. 
  • Under the direction of their supervisor, will lead in clinical data management for federal, state, and private projects/grants; ensures that reports and deliverables are accurate and that timelines are met such as UDS, MCO and PCMH.
  • Participate in the planning, development, presentation, and delivery of education programs in support of NCQA PCMH standards. 
  • Under the direction of their supervisor, will develop and support the execution and completion of the annual Primary Care QI Plan.
  • Promote workflow redesign and structured data entry to improve care coordination, data reporting and analysis. 
  • Collaborate with senior and site leadership as well as key committees to develop long-range goals, objectives, and strategies for quality/safety improvement across the network. 
  • Conduct on-site/virtual visits to assess and assist care teams through changes to workplace culture, efficiency, documentation, and use of technology needed to adapt to continuous changes in healthcare standards and regulations. 
  • Prepare monthly QI committee minutes. 
  • Participate in assigned standing committees and improvement teams. 
  • Attend all meetings and/or training sessions as required.
  • Co-facilitate the Network-wide IHI Collaborative. 
  • Liaison with Azara to ensure integrity of CPCI data.
  • Stay abreast of FQHC regulatory body quality and value-based care standards, including BPHC, HRSA, NCQA, and CMS.
  • Remain up to date on healthcare policies.
  • Execute other tasks as assigned.

Requirements

  • Bachelor’s Degree required.
  • Master’s degree in public health, Public Administration, Urban Planning, or related field prefeed.
  • Familiarity with Healthcare Effectiveness Data and Information Set (HEDIS) measures prefeed.
  • Experience in community-based settings prefeed.
  • Bilingual: English/Spanish.
  • One year of experience in a health care or not-for-profit position.
  • Experience in data analysis.  
  • Excellent quality assurance/quality control, analytical and ability to cull and interpret data.
  • Excellent research, writing and communication skills.
  • Ability to work collaboratively with other staff, specifically program staff.
  • Ability to manage several projects simultaneously and meet strict deadlines.
  • Ability to organize and package information in a concise and attractive manner.
  • Familiarity with Microsoft Office Suite.


Preferred Qualifications:

  • Master’s degree in public health, Public Administration, Urban Planning, or related field prefeed.
  • Familiarity with Healthcare Effectiveness Data and Information Set (HEDIS) measures prefeed.
  • Experience in community-based settings prefeed.
  • Bilingual: English/Spanish.
