Mental Health Counselor
We are seeking a highly skilled Mental Health Counselor to join our team at Promise Healthcare's health center, providing a range of care services, including primary medical care, behavioral health services, and on-site dental services.
Primary Responsibilities:
- Deliver culturally competent psychotherapy or counseling services to individuals, groups, or families.
- Conduct thorough assessments to identify patients' social and emotional needs and develop targeted interventions.
- Collaborate with multidisciplinary teams to ensure seamless delivery of integrated care services.
Detailed Duties:
- Assess patient referrals and develop mental health treatment plans.
- Provide timely direct counseling services to patients.
- Monitor patients' progress and adjust treatment plans as necessary.
- Maintain accurate records in our electronic health record system related to counseling, collaboration, and patient interactions.
- Present case consultations and update treatment plans as required.
- Coordinate referrals for specialized services and track progress.
- Collaborate with medical and social services providers to address patients' emotional and psychological needs.
- Advocate for improved services for patients with mental health needs.
- Contribute to designing and implementing an effective mental health program within Promise Healthcare.
Minimum Requirements:
- Exceptional communication and interpersonal skills.
- Familiarity with local medical and social service systems.
- Strong documentation skills and electronic health record management.
- Ability to work independently and collaboratively in a dynamic clinic environment.
