Mental Health Counselor
We seek a skilled Mental Health Counselor to join our team at Promise Healthcare's health center, providing comprehensive care services, including primary medical care, behavioral health services, and on-site dental services.
Responsibilities:
- Develop and implement individualized therapy plans to meet patients' unique needs.
- Conduct thorough assessments to identify patients' social and emotional needs and develop targeted interventions.
- Collaborate with multidisciplinary teams to ensure seamless delivery of integrated care services.
Key Duties:
- Assess patient referrals and develop mental health treatment plans.
- Provide direct counseling services to patients promptly.
- Monitor patients' progress and adjust treatment plans as necessary.
- Maintain accurate records in our electronic health record system related to counseling, collaboration, and patient interactions.
- Present case consultations and update treatment plans as required.
- Coordinate referrals for specialized services and track progress.
- Collaborate with medical and social services providers to address patients' emotional and psychological needs.
- Advocate for improved services for patients with mental health needs.
- Contribute to developing an effective mental health program within Promise Healthcare.
Requirements:
