What are the responsibilities and job description for the Home Care Liaison/Marketing position at Promise Home Care Services?
Responsibilities:
Develop and maintain relationships with referral sources (hospitals, case managers, discharge planners, social workers, etc.)
Educate the community and professionals about our services and programs (CHC, OBRA, Act 150)
Attend networking events, health fairs, and community outreach programs
Meet and exceed monthly referral and enrollment goals
Assist new clients and families during the onboarding process
Maintain accurate records of contacts and follow-ups
Provide feedback to management on referral trends and opportunities
Requirements:
Previous experience in homecare, healthcare marketing, or community outreach preferred
Knowledge of Medicaid waiver programs (CHC, OBRA, Act 150) is a plus
Strong communication, interpersonal, and presentation skills
Reliable transportation and valid driver’s license
Highly organized and self-motivated
