Job Summary
PromptCare is seeking a highly skilled and compassionate In-Home Infusion Nurse Specialist to join our team. This role requires the ability to provide high-quality care to patients in their own homes, working closely with patients, families, and healthcare teams to facilitate care coordination and patient discharge.
About the Role
- Conduct initial nursing assessments and evaluations to qualify patients for home infusion therapy.
- Train patients and caregivers in procedures and equipment, ensuring they have the necessary skills to manage their treatment effectively.
- Provide timely and accurate information to patients and family members, addressing any concerns or questions they may have.
- Develop and revise patient plans of care with the patient, family, and healthcare team, taking into account the patient's unique needs and circumstances.
Requirements and Qualifications
- Registered Nurse in New York State with a license in good standing.
- Valid driver's license and automobile insurance.
- CPR Certification.
- At least 2 years of experience as a home infusion nurse or equivalent experience.
