Axis HealthCare is looking for someone to join our chronic care management team!
This role is more than just taking care of patients, it's speaking life and encouragement to the patients that you talk to.

The majority of this role would be spent on the phone with patients. While on the phone you would be doing the following:
- following up on their chronic conditions
- seeing how they are doing
- checking on referrals for them
- if they need a provider visit
- meeting their needs
- going above and beyond for all their care
- following up on hospital visits

We prefer someone that has a license or certification but it is not required. Medical experience of 1 years is required.
