As a Medical Social Worker at Prophylaxis HealthCare, you will play a vital role in supporting patients through their healthcare journey. Responsibilities
- Conduct assessments to evaluate patients' social and emotional needs.
- Develop and implement individualized care plans in collaboration with healthcare teams.
- Provide crisis intervention and support for patients facing challenging situations.
- Facilitate access to community resources and services for patients and families.
- Maintain accurate documentation in compliance with HIPAA regulations.
- Educate patients and families about available resources and support systems.
- Participate in clinical research initiatives as needed to enhance patient care.
- Advocate for patients' rights and needs within the healthcare system.
Qualifications
- Master's degree in Social Work (MSW) from an accredited program.
- Valid state licensure as a clinical social worker (LCSW) or equivalent certification.
- Proven experience in case management or medical social work settings.
- Strong knowledge of medical terminology and ICD-9 coding is preferred.
- Demonstrated skills in crisis intervention and psychotherapy techniques.
- Familiarity with assisted living environments is a plus.
- Experience working with children or families is beneficial but not required.
- Excellent communication skills and ability to work collaboratively with healthcare professionals.
