We are seeking a dedicated and compassionate Case Manager to join our substance abuse treatment company. The Case Manager will be responsible for coordinating care for individuals struggling with substance use disorders, ensuring they receive the necessary support, resources, and treatment services.
Schedule: Monday - Friday, 9:00am - 5:00pm
Key Responsibilities:
- Conduct client assessments to determine needs and develop individualized treatment plans.
- Provide referrals to medical, mental health, housing, employment, and other community resources.
- Monitor client progress and adjust care plans as needed.
- Facilitate weekly individual meetings with clients to assess progress, address challenges, and provide ongoing support.
- Collaborate with therapists, medical professionals, and support staff to facilitate comprehensive care.
- Develop and implement discharge planning, ensuring clients transition smoothly into aftercare programs, community resources, or supportive environments.
- Maintain accurate documentation and ensure compliance with confidentiality and regulatory standards.
- Advocate for clients to ensure access to essential services and support systems.
Qualifications:
- Bachelor’s degree in social work, psychology, or a related field (Master’s preferred).
- Experience in case management or working with individuals affected by substance abuse.
- Knowledge of addiction treatment resources and community services.
- Strong communication, organizational, and advocacy skills.
- Ability to work effectively in a team-oriented environment.
Job Type: Full-time
Pay: $50,000.00 - $60,000.00 per year
