Credentialing takes 30-60 days (six weeks is standard for Locums who submit app. / documents timely)
The following immunizations are required for credentialing (before the start date) : Flu Vaccine,
TB Status / X-Ray Results (results must be within 90 days before the start date), Positive MMR Titer or
Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella
Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years),
HCO & Vendor agree to a 30-day notice for ending all assignments.
