No guaranteed hours, FT available
COTA will support all service areas under branch as needed
Prior HH experience as well as HCHB experience highly preferred
Position Summary:
Assists licensed/registered/certified Occupational Therapist in administration of occupational therapy programs in compliance with state practice act(s). Provides therapy designed to maintain or restore the patient's ability to function and in a manner which complies with Gentiva Health Services policies and procedures, applicable healthcare standards and governmental laws and regulations. Works under moderate supervision.
Primary Responsibilities
1. Abides by and demonstrates the company Mission - Vision - Values through both behavior and job performance on a day-to-day basis.
2. Provides therapy services planned, delegated and supervised by the qualified Occupational Therapist in accordance with the patient's Plan of Treatment.
3. Assists in the implementation of vocational/education programs and activities established by registered Occupational Therapist designed to restore, reinforce, and enhance task performances, diminish or correct pathology, and to promote/maintain health and self-sufficiency.
4. Designs/adapts equipment and working and/or living environment. Fabricates devices to assist and improve function and independence and participation in the program and/or community where possible.
5. Provides therapeutic treatment and instruction to patients as directed by the qualified Occupational Therapist and in accordance with physician orders to improve/restore strength, coordination, range-of-motion and function or teach compensation measures.
6. Reports information and observations to Occupational Therapist and/or Clinical supervisor, documents observed information in patient records and prepares clinical notes. Assists with preparation of progress reports. Maintains and submits documentation as required by the Company and/or facility.
7. Instructs patients and family members regarding home programs as well as care and use of adaptive equipment.
8. Maintains the highest standards of professional conduct in relation to information that is confidential in nature. Shares information only when recipient's right to access is clearly established and sharing of such information is in the best interests of the patient.
9. Participates in care coordination and discharge planning activities and acts as a resource to other health care personnel in meeting patient's needs.
10. Designs community reintegration activities, as appropriate, to assist the client in the physical reconditioning effort, and/or the psychological adjustment and coordinates the plan with members of the interdisciplinary team.
11. Maintains/enhances clinical practice skills by attending, participating in/conducting staff development/ continuing education programs.
12. Participates in Performance Improvement activities as assigned. Communicates information that enables collection and root cause analysis of data to identify opportunities for improvement.
13. Adheres to and participates in Company's mandatory HIPAA privacy program/practices and Business Ethics and Compliance programs/practices.
14. Reviews and adheres to all Company policies and procedures and the Employee Handbook.
15. Participates in special projects and performs other duties as assigned.
Skills
Basic knowledge of rehabilitation techniques related to complex neurological injury - Required
Current CPR certification - Required
Good organizational and communication skills and the ability to function with minimal supervision - Required
Minimum 6 months experience - Required
Current licensure/certification in the state of practice - Required
Education
Graduated after successful completion of an occupational therapy assistant education program accredited by the Accreditation Council for Occupational Therapy Education, (ACOTE) of the American Occupational Therapy Association, Inc. (AOTA) or its successor organizations.
