Manager of Case Management- Fort Pierce, FL
Job Type: Full-time/Permanent
Schedule: Monday- Friday (No Weekends)
Job Summary:
The Manager for Case Management services reports to the Case Management Director and is responsible for promoting patient-centered care by coordinating all aspects of hospital-based case management for his/her assigned area of responsibility in alignment with the goals of the Case Management Department.
Qualifications/Requirements:
Associate's/ Bachelor's Degree in Nursing OR Master's Degree in Social Work
3 years' experience in Acute-Care Case Management or Social Work
2 years of supervisory experience
