Physical Therapist (PT) - Coldwater, MI.
Job Type: Full-Time.
Shifts: Monday through Friday from 7:30 AM to 04:00 PM
Summary
ProTouch Staffing has partnered with an established Occupational Health client located in Coldwater, MI that is seeking a qualified Physical Therapist for one of their processing facilities. This is a full-time direct-hire opportunity for candidates that have at least 1 to 2 years of experience and are looking to step into an outpatient role as a Physical Therapist. The top priority of the Physical Therapist is to perform physical therapy evaluation and create personalized treatment plans for team members with injuries or illnesses, supervise physical therapist assistants (PTA)when appropriate, and complete HPEs. The Physical Therapist will also be asked to perform administrative tasks and scheduling to ensure efficient and effective operations of the physical therapy clinic.
Communicate with supervisors via phone and in-person about patient restrictions
Collaborate with ATC and Case Manager about Plan of Care (POC) for patients
Complete Human Performance Evaluations (once HPE training is completed.)

- Under direct supervision of the Clinical Director, the Physical Therapist works within the partner company guidelines, practice standards for their state and professional organization, and our company's policies and procedures.
- Physical Therapy Initial Evaluation for workers compensation and/or Direct Access
- Documentation of such evaluation
- Physical Therapy Treatment
- Documentation of treatment sessions on treatment cards
- Develop HEP for patients, using HEP2GO website and print copies for patients
- Documentation of weekly progress note for each patient in OHM
- Email weekly progress notes for each patient to PMA and Case Manager
- Complete OHU form whenever patient status changes
- Scan all OHU forms into the Infirmary Drive
- At physical therapy discharge:
- Scan Treatment Card to Infirmary drive
- Subsequently attach all Treatment Cards to appropriate encounter on OHM
- Bring hard copies of Treatment Card and OHU forms to First Aid to be filed

Weekly Responsibilites
- Complete physical therapy statistic
- Contact clinic manager about needed supplies
- Weekly case-management meeting to review each patient with staff
- Call referring providers about protocol and POC (when referred by specialists)
- Trigger Staff/Temporary staff onboarding
- Trigger staff billing/SuperBill
- Consult for ergonomic assessment on the floor
- Create onboarding/SOPs
- Doctor of Physical Therapy (DPT) or Master's in Physical Therapy (MPT) from an accredited program.
- 1-2 years of experience as a Physical Therapist.
- Current Michigan state licensure as a Physical Therapist.
- Current BLS certification.
- Dental, health, vision, CME, PTO, etc. - These will be discussed in more detail during your interview.
