- Conduct routine and acute patient evaluations in skilled nursing and assisted living settings.
- Develop and manage comprehensive treatment plans for chronic and acute conditions.
- Collaborate with facility staff, nurses, and other healthcare providers to coordinate patient care.
- Provide education and guidance to patients and families on treatment options and disease management.
