RN Case Manager

Job Summary
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. The ideal candidate will be responsible for coordinating patient care, ensuring compliance with healthcare regulations, and providing support to patients with a focus on infectious disease care. This role requires a strong understanding of managed care principles, critical care practices, and effective patient service strategies. The Registered Nurse Case Manager will play a vital role in enhancing patient outcomes through comprehensive case management.

Responsibilities

• Develop and implement individualized care plans for patients, focusing on their unique needs and health conditions.
• Conduct thorough assessments of patients’ medical histories and current health status.
• Coordinate with multidisciplinary teams to ensure continuity of care and effective communication among healthcare providers.
• Educate patients and their families about disease management, treatment options, and available resources.
• Monitor patient progress and adjust care plans as necessary to achieve optimal health outcomes.
• Maintain accurate documentation in compliance with HIPAA regulations and organizational policies.
• Utilize medical terminology effectively to communicate with healthcare professionals and document patient interactions.
• Advocate for patients’ needs within the healthcare system, ensuring they receive appropriate services.

Experience

• Current Registered Nurse (RN) license in the state of practice.
• Experience in case management or a related field is preferred.
• Knowledge of infectious disease care and critical care practices is highly desirable.
• Familiarity with managed care principles and patient service excellence.
• Strong understanding of physiology knowledge as it pertains to patient care.
• Excellent communication skills, both verbal and written, with the ability to educate patients effectively.
• Proficiency in medical terminology relevant to nursing practice.

Pay: From $2,000.00 per month
