Responsibilities
- Develop and maintain individualized care plans for residents, ensuring they align with their needs and preferences.
- Coordinate with healthcare providers, families, and staff to facilitate effective communication regarding resident care.
- Monitor residents’ health status and make necessary adjustments to care plans as needed.
- Provide support in behavior management techniques to enhance resident engagement and comfort.
- Educate staff on resident rights and ensure compliance with relevant regulations.
- Assist in training new caregivers on best practices in home care and dementia care.
- Maintain accurate documentation of resident care activities, including any changes in condition or behavior.
- Conduct regular assessments of the living environment to ensure cleanliness and safety for all residents.

Requirements

- Certification in First Aid is a plus.
