Position Summary
This position is responsible for providing referral services to home care clients, families and caregivers, addressing issues such as adjustment to illness/prognosis, care system issues, community resource issues, and insurance/financial issues. The MSW develops and implements a plan of care in collaboration with the Home Care team providing care to the patient.
Position Specific Responsibilities
- Provides medical social work services to patients and families including assessment, counseling, casework, community resource referral, advocacy and discharge planning utilizing current practice knowledge and skill.
- Consults and educates staff, volunteers and community of the social and emotional factors related to the care of the terminally ill patient and the family.
- Provides social work in accordance with the NASW Code of Ethics and Providence Hospice and Home Care philosophy, policies, and procedures.
- All required documentation will be complete, accurate and timely.
- Promotes and practices patient/family, personal and environmental safety while performing job duties according to agency policies and procedures.
- Participates in relevant educational activities to provide competent care to patients/caregivers, to improve the social work skills and to seek personal and professional growth.
- Displays professionalism in the work environment in all areas including communication with both internal and external customers.
- Complies with all applicable Federal, State, and local laws, regulations, and CHAP requirements as well as facility/agency specific policies and procedures.
- Maintain confidentiality of all client demographics, medical and financial information at all times.
- Maintain confidentiality of all company and client information.
- Ensure proper handling and disposal of confidential documents and adheres to all HIPAA rules and regulations.
- Adheres to all policies and procedures including regulatory requirements with regard to infection control, practicing standard precautions, sanitation, and safety and disaster preparedness.
Education and/or Experience
- Required - Master's Degree Social Work
- Required - 1 year Health care setting.
- Preferred - 2 years Health care setting.
- Preferred - Home health care setting.
Skills
- Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
- Language Skills: Communication:
- Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.

Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Compensation range: $30.98-$48.22 with bonus eligibility (10% short term incentive)
The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities
