This posting is for multiple openings of RN - Advanced Heart Failure at Providence Sacred Heart Medical Center & Children's Hospital in Spokane, WA. We offer day shifts for full-time exempt opportunities.
Provides coordination / case management for heart failure patients across the continuum of care. Facilitates patient care through heart failure clinic with a strong focus on outpatient follow-up. Utilizes multidisciplinary team approach to organize patient care, manage complex cases, and prevent readmissions. Provides professional, peer and patient education and participates in the development of patient education programs.
