What are the responsibilities and job description for the RN Hospice Coordinator position at Providence Careers?
Description
This position facilitates the intake processes for the Hospice. This includes providing information, consultation, and assistance for all inquiries regarding home health and hospice services; matching client needs with agency services and reimbursement sources; and coordinating with agency staff for initial client assessment visit.

Required Qualifications :
- Graduate of an accredited / approved school of Nursing
- Upon hire : Alaska Registered Nurse License

About the Team
Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.
HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses.
