Job Summary

Responsibilities
- Conduct thorough physical examinations and assess patients' health status.
- Develop and implement individualized patient care plans, including discharge planning.
- Administer medications and manage pain effectively for patients.
- Perform specimen collection processing and ensure proper handling and documentation.
- Utilize electronic health record systems such as Cerner and Athenahealth for patient management.
- Engage in utilization review to optimize patient outcomes and resource use.
- Provide education to patients regarding health maintenance, disease prevention, and treatment options.
- Collaborate with interdisciplinary teams to coordinate comprehensive care, including tube feeding when necessary.
- Monitor patients undergoing dialysis and provide appropriate interventions as needed.
Qualifications
- Master’s degree in Nursing with a focus on Family Nurse Practitioner training.
- Current state licensure as a Family Nurse Practitioner.
- Strong clinical skills in physical examinations, medication administration, and pain management.
- Experience or familiarity with discharge planning, specimen collection processing, and utilization review is preferred.
- Proficiency in electronic health record systems such as Cerner or Athenahealth is advantageous.
- Excellent communication skills with the ability to educate patients effectively.
