SUMMARY:
Thorough knowledge of general nursing principles and procedures and understanding of the concepts of aging and the needs and concerns of the elderly population. Knowledge of state and federal health regulations and clinical standards of nursing practice and delivery systems, supervision management, staff development and cost containment. Ability to perform comprehensive assessments and development individualized plans of care based on resident needs. Ability to communicate effectively, verbally and in writing with all levels of health care professionals and support staff. Must be able to work under varying degrees of supervision. Display positive professional image and enhance positive public relations for the facility. The primary purpose of this position is to provide high quality, holistic nursing care while also maintaining medical records. Lead or direct licensed and non-professional staff in the delivery of direct resident care and support functions.
A. Manage communication and relationships between CNAs, charge nurse & management
B. Provide after hour/weekend support to facility as nurse on-call
C. Attend meetings as required on and/or off campus representing PHC
D. Assists nursing staff with admissions as needed
E. Assists with wound care as needed
F. Complete mandatory reports/forms
G. Monitors and replace outdated medications while adhering to proper procedure
H. Assures that refrigerator temperature logs are being completed timely
I. Adheres to safety and infection control policies and procedures
J. Communicate effectively orally and in writing
K. Maintain unit and work area in clean safe and orderly manner
L. Promote positive public relation for the facility
M. Perform other tasks as required
N. Able to cover both units as needed
Manage communication and relationships between CNAs, charge nurse & management
- Monitor information flow from nurse to CNA.
- Prevent hostility/misunderstandings between employees.
- Report misbehavior to management & human resources when necessary.
Provide after hour/weekend support to facility as nurse on-call
- Assist to find solution for staff member.
- Must be available entire time while on-call.
Attend meetings as required
- Attend weekly care plan meetings as unit representative.
- Attend monthly gradual dose reduction meetings as unit representative.
- Attend daily clinical and stand-up meetings.
- Attend monthly fall committee meetings and provide follow-ups.
- Attend monthly safety committee meetings and provide follow-ups.
- Any other meetings as needed
Assists nursing staff with admissions as needed and audit completeness and accuracy
- Assure that room is prepared and ready for resident
- Assist with entering any standing orders as needed
Assists with wound care as needed
- Fill in for wound nurse if absent
- Assist wound nurse if needed
Complete mandatory reports/forms
- Review 24 hour reports daily.
- Complete/review shower schedule.
- Complete 672, 802 forms.
- Review alarm sign—off sheets.
- Complete CNA assignment sheets.
- Monitor Coumadin doses.
Monitors and replace outdated medications while adhering to proper procedure
- Check dates on medications weekly
- Dispose of any outdated medications using proper procedure
Assures that refrigerator temperature logs are being completed timely
- Audit refrigerator temperature logs weekly
Adhere To Safety and Infection Control Policies and Procedures
- Maintain good personal hygiene and adhere to uniform requirement.
- Practice standard precautions.
- Audit cleaning of resident bedside unit, care equipment, and utility room
- Provides for the safety of the resident using safety devices identified on the assignment sheet.
- Report personal illness or skin lesions.
- Participates in orientation training and twelve (12) hours of in-service programs annually.
- Report defective equipment or other potential safety hazards promptly.
- Adheres to Nursing Department policies and procedures.
- Monitors Side Rail Compliance
Perform Other Tasks as Required
- Be able to complete documentation in the electronic medical record.
- Assist other personnel when assignment completed.
- Knows and follows existing lines of communication and authority.
- Other - as designated by Director of Nursing.
- Assures that resident rights are maintained at all times, reporting suspected deviations to immediate supervisor.
QUALIFICATIONS
- Ability to read/write comprehensive English and document observations.
- Must perform basic math.
- Ability to verbally communicate with residents, staff and general public in a manner that is understandable, must be honest, dependable and exhibit a warm, cheerful, caring manner.
- Must be able to safely perform the essential functions of the job with or without reasonable accommodation.
EDUCATION and/or EXPERIENCE
- Current RN License in good standing on file with the Indiana Nursing Board.
- 5 years’ experience in long-term care or other health care setting.
PHYSICAL DEMANDS
Physical
- Will need to continuously stand and walk, squat, bend to floor, reach, twist, sit and potentially remain in uncomfortable positions for extended periods of time.
- Must be able to lift, move and turn residents. Requires varying degrees of pushing, pulling and lifting depending on the weight of the resident, which is often times over 100 pounds.
- Must have eye/hand coordination sufficient to assist residents with ADL's, transfers, and ambulation.
- Must be able to clearly speak.
- Must deal calmly with multiple tasks, noises, and interruptions on a continuous basis.
