Description
Under minimum supervision the CARE Network Social Worker-MSW works collaboratively in a multidisciplinary team providing community based comprehensive case management services for special populations. The MSW Social Worker position requires independent functioning, accurate assessment skills, and direct communication with clients, and caregivers. The Social Worker uses knowledge of psychosocial assessment tools, depression screenings, health plan guidelines and community resources in planning and coordinating care for clients. Efficiently communicates with physicians, customers, caregivers and partner agencies in person, in writing and through use of diverse HIPAA compliant electronic communication platforms. Initiative and the ability to make independent judgments are required.

Required Qualifications:
- Master's Degree in Social Work or equivalent education/experience.
- 5 years of experience in community social services case management; Or
- 3 years of experience in hospital social services with 1 year of experience in community social services.
