PHYSICAL THERAPIST – HOME HEALTH
Schedule: Full time Day shift
Possible Service Locations: Hesperia CA
Pay Structure: Pay Per Visit
Job Description:
Physical Therapists develop, coordinate and maintain therapeutic services and activities alongside an interdisciplinary team to help patients in need successfully maintain or improve their level of independence.

Required qualifications:
- Master's degree in physical therapy, Or
- Degree in Physical Therapy from an approved program according to Medicare guidelines
- California Physical Therapist License upon hire
- National Provider BLS - American Heart Association within 30 days upon hire
Preferred qualifications:
- Home Health experience.
- 1 year of experience as a Physical Therapist.

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.
HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses.
