Description
Provides psychosocial assessment and delivery of counseling, social work and emotional support services to terminally-ill patients, and/or palliative services to patients and their families. Works as a team member to identify problems and assist patient/family members in exploring realistic options, decision making and problem solving.

Required qualifications:
- Master's Degree in Social Work, Psychology or Counseling. Graduate from an accredited school. or Ph.D. in Social Work, Psychology or Counseling. Graduate from an accredited school.
- Coursework/Training: Training in family counseling and case management.
- California Clinical Social Worker License upon hire or California Marriage and Family Therapist License upon hire.
- 1 year Experience in a health care setting or related field.
