Job Description

Job Title : Physical Therapist (PT) - Home Health

Job Specialty : Home Health

Job Duration : 13 weeks

Shift : Day, 5x8-Hour (08 : 00 - 16 : 30)

Guaranteed Hours : Facility can call off up to one shift per pay period.

Experience : 1 year of skilled home health experience required; First-timers not accepted.

License : Pending license accepted; confirm with the hiring manager.

Certifications : BCLS required.

Must-Have : Reliable transportation required; Oasis experience preferred but not required.

Job Description :

• Provide home health physical therapy services in Medford, OR, and surrounding areas.
• Manage patient load with a standard of 30 productivity points, visiting up to 6 patients per day.
• Strict adherence to a 1 : 1 patient ratio.
• Engage in 40 hours of non-billable orientation.
• Expected to complete modules pre-start and annually; average of 16 hours.
• Maintain accurate patient records using EPIC charting system.
• Ensure adherence to specific shift requirements and cannot accommodate 4x10s or 3x12s shifts.
• No weekend or on-call requirements.
• Must comply with COVID-19 and flu vaccine policies.
• Free choice of scrub colors.
• Mileage reimbursed according to IRS standards.
• Maintain excellent communication and coordination with the healthcare team.
• Ability to float between all facilities within a given state is expected.

Please note : This position does not permit the accommodation of block schedules or local rate differences within a 50-75 mile radius. If a communication device is required for assignment duties, and not provided, submission of monthly phone usage for work-related purposes is expected.
