Providence at Home is a growing division of Providence Life Services, which provides a continuum of healthcare, focused on senior living. Providence Hospice is seeking a Registered Nurse (RN) Quality Improvement Coordinator in Homer Glen, IL. Expected position hours are Monday thru Friday, 8:00AM - 4:30PM, exempt/salaried.

This position reports directly to the VP of Home and Community Based Services for Providence at Home and will be based out of the Homer Glen office (even though responsibilities extend to Indiana as well). The position aids in the oversight of quality programs for the entire Home and Community Based Services division, which includes Home Health, Hospice and Private Duty in both Illinois and Indiana.
- Implement, monitor and provide direction in the organization’s quality improvement activities and programs.
- Coordinates activities related to clinical record review for data collection. Oversees peer review and all audit activities of the organization.
- Compiles, trends and reports quality data in the following areas: patient care, safety, risk management, infection control, outcomes, and customer satisfaction. Data is analyzed and compared internally and externally with other sources when available.
- Establishes a process model for improvement and provides education and training to organization personnel on the performance improvement plan, and team development. Monitors patient satisfaction outcomes and collaborates with management staff in follow-up of concerns expressed by patients, families, and physicians.
- Develops, implements, and keeps records of programs to ensure employee orientation, competency and in-service education meet the needs of the organization.
- Proficient in use of EMR and assists employees with orientation to the EMR system.
- Maintains current knowledge of Medicare, State and Joint Commission regulations and ensures that the organization is following regulatory guidelines.
- Maintains current knowledge of home safety and infection control guidelines.
- Responsible for ensuring processes to monitor and evaluate safety, risk management and infection control programs. Works collaboratively to ensure documentation is complete and consistent with care that reflects regulatory guidelines.
- Monitor organization compliance with regulatory and accreditation standards.
- Maintains current knowledge related to Medicare Conditions of Participation and the Joint Commission’s standards to ensure that the organization is compliant with state, federal and accreditation guidelines.
- Coordinates and acts as liaison for state, federal and Joint Commission audits and surveys.
- Develops and demonstrates proficiency with use of information technology, agency clinical documentation system and related quality software.
- Maintains knowledge of local, state and federal regulatory requirements and guidelines, inclusive of, but not limited to Joint Commission, CMS/Medicare, OSHA, CDC, and OIG guidelines.
Requirements
- Licensed Registered Nurse in the state of Illinois
- Licensed Registered Nurse in the state of Indiana preferred.
- Homecare and/or Hospice experience required.
- Quality Assurance/Improvement experience required.
