The Care Manager RN is responsible for coordinating care management services for at-risk patients with complex medical and psychosocial needs. Description
The Care Manager RN provides care management and care coordination services for at-risk patients, including patients with complex medical and psychosocial issues. Population-based, chronic care management approaches and application of clinical protocols are integral to the program. This position is a core member of the patient-centered medical home (PCMH) teams led by the physician/provider.
Key Responsibilities
- Coordinate care management services for at-risk patients with complex medical and psychosocial needs.
- Develop and implement population-based care plans that address the unique needs of each patient.
- Collaborate with healthcare providers and other stakeholders to ensure seamless care coordination and communication.
- Maintain accurate and timely records of patient care activities and outcomes.
Required Qualifications
- Graduation from an accredited school of nursing.
- Oregon Registered Nurse License.
- 3 years Clinical experience as an RN.
Preferred Qualifications
- Bachelor's Degree in Nursing.
- 2 years Home health, skilled nursing facility and/or community health nursing.
- 2 years Managed care experience and/or utilization management experience.
- 2 years Care/Case Management experience.
