Clinical Liaison RN (Part Time)
As a Clinical Liaison RN, this part-time role emphasizes the coordination of home care services between Providence Home Services and contracted facilities. With a focus on patient-centered care and collaboration, the Clinical Liaison ensures that home health, home infusion, and medical equipment services are integrated into patient discharge plans.
Description
This is a part-time (28 hours/week) position and it will be floating between PPMC and PSVMC.
This position represents Providence Home Services (HS) at contracted facilities and requires strong communication and interpersonal skills. The role of the Clinical Liaison is to coordinate and arrange home care services between HS and contracted facilities for home health, home infusion, and home medical equipment associated with a clinical referral, except when there is an onsite HME liaison.
The position acts as a clinical resource and educates hospital staff regarding the services offered by HS, and contributes to the patient care plan as it relates to the discharge plan. The Clinical Liaison provides information to ensure a smooth transition for patients and their families following hospitalization.

Required qualifications:
- Oregon Registered Nurse License
- National Provider BLS - American Heart Association
- In compliance with the Providence Health and Services-OR Human Resources policy #302 Background Checks, a criminal background check is required for this position, based on assigned duties
Preferred qualifications:
- Bachelor's Degree in Nursing
- 1 year of Full-time experience in home health or hospice
- 1 year of Full-time experience in home infusion
- 1 year of Full-time experience in discharge planning or care management
- Experience in home care and in positions that required extensive customer service skills
