Under the direction of Home Health Clinical Supervision and on order of the physician, this position is accountable for direct and indirect quality care for patients. The successful candidate will be responsible for coordinating patient care activities with other members of the health care team in a collegial relationship.
Key Responsibilities
* Provide quality patient care services in accordance with the Nurse Practice Act
* Coordinate patient care activities with other healthcare professionals
* Collaborate with the healthcare team to ensure seamless delivery of care
* Ensure accurate documentation of patient information and communication with the healthcare team
Requirements
* Education to meet certification, license or registration requirement
* 1 year Medical/surgical experience in an acute care setting
* 2 years Experience in acute care or community mental health setting for Mental Health RN work assignment
