Job Description:
The Registered Nurse will provide direct and indirect quality care for patients in their homes, following the nursing process and in accordance with Oregon State Board of Nurse Practice Act. The ideal candidate will possess excellent communication and interpersonal skills, as well as the ability to work independently and collaboratively as part of a multidisciplinary team.
- Develop and implement individualized care plans that address the physical, emotional, and social needs of patients
- Provide education and support to patients and their families on disease management, self-care techniques, and other health-related topics
- Collaborate with other healthcare professionals to ensure seamless transitions of care and coordination of services
- Maintain accurate and timely documentation of patient progress and outcomes
Required Skills and Qualifications:
To be successful in this role, you will need:
- A current Oregon Registered Nurse License
- 1 year of medical/surgical experience in an acute care setting required for Home Health RN
- 2 years of pediatric experience in an acute care setting for RN caring for pediatric patients
- Excellent communication and interpersonal skills
- Ability to work independently and collaboratively as part of a multidisciplinary team
