Job Description:
We are seeking a skilled and compassionate Home Health RN to join our team at Providence Benedictine Nursing Center. As a Home Health RN, you will be responsible for providing high-quality patient care in the comfort of their own homes.
About the Role:
You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient. Your expertise will enable you to provide holistic care that addresses the physical, emotional, and social needs of our patients.
Key Responsibilities:
- Conduct comprehensive assessments of patients to identify their needs and develop personalized care plans.
- Provide direct patient care services, including wound care, medication management, and education on self-care techniques.
- Collaborate with other healthcare professionals to coordinate care and ensure seamless transitions between settings.
- Maintain accurate and timely documentation of patient interactions, including progress notes and care plans.
Requirements:
- Oregon Registered Nurse License
- National Provider BLS - American Heart Association
- 1 year of Medical/surgical experience in an acute care setting
- 2 years of experience in acute care or community mental health setting for Mental Health RN work assignment
Preferred Qualifications:
- Recent Home Health experience
- Hospice or Palliative Care experience for Palliative Care RN work assignment
- Point of Care experience
