Job Description
This position will be traveling to see patients primarily in the Portland area.
The Hospice Nurse, under physician direction, completes medical requirements while coordinating a whole-person plan of care. The position is accountable for direct and indirect quality care for patients of all ages, according to individualized needs, following the nursing process, and in accordance with the Nurse Practice Act.
The position accepts professional development as an integral part of nursing practice. It coordinates patient care activities with other members of the healthcare team in a collegial relationship. The position upholds the standards of confidentiality, attendance, and punctuality.
Responsibilities:
- Provide high-quality, patient-centered care to individuals of all ages.
- Develop and implement individualized plans of care that address the physical, emotional, and spiritual needs of patients.
- Coordinate care with other healthcare professionals to ensure seamless transitions and optimal outcomes.
- Maintain accurate and detailed records of patient interactions and interventions.
- Stay current with best practices and research in hospice and palliative care through ongoing education and training.
Qualifications:
- Education to meet certification, license, or registration requirement.
- Oregon Registered Nurse License upon hire.
- National Provider BLS - American Heart Association upon hire.
- Washington Registered Nurse License upon hire or obtain within 90 days of hire.
- 2 years of Medical/surgical experience in an acute care setting, and/or a combination of medical/surgical, long-term care, or home health or hospice experience.
- Experience using electronic medical record software.
