We are seeking an experienced and compassionate Hospice RN to join our team at Providence Home Services as a Geriatric Care Consultant. In this role, you will have the opportunity to use your expertise to support older adults and their families, providing guidance on end-of-life care and helping them navigate complex healthcare systems.
Responsibilities
- Assess patient needs and develop tailored care plans in collaboration with interdisciplinary teams
- Deliver educational programs and workshops on geriatric care and end-of-life issues
- Collaborate with patients, families, and healthcare providers to ensure seamless care coordination
- Maintain accurate records and contribute to quality improvement initiatives
Requirements
- Certified as an RN or LPN/LVN in Oregon; Washington RN license upon hire or within 90 days
- Minimum 2 years of experience in hospice, home health, or acute care settings
- Electronic medical record software proficiency
- Strong communication and interpersonal skills
