Job Summary:

• Provide comprehensive nursing care to patients in their homes, focusing on prevention, early intervention, and education
• Collaborate with the interdisciplinary care team to assess patient needs and develop individualized care plans
• Document patient progress and outcomes in accordance with regulatory requirements

Education and Experience:

• BSN or higher from an accredited institution
• Oregon Registered Nurse License upon hire
• National Provider BLS - American Heart Association certification within 30 days of hire
• Minimum two years of experience in acute care settings
