Hospice Care Coordinator Job Summary

This full-time day shift position involves working as an office-based nurse, with weekend/holiday rotation coverage. The ideal candidate will have strong leadership skills and the ability to coordinate care plans for patients of all ages.

Duties and Responsibilities

  • Under physician direction, complete medical requirements and coordinate whole-person plans of care
  • Provide direct and indirect quality care for patients of all ages

Requirements

  • Education to meet certification, license or registration requirement
  • Oregon Registered Nurse License (upon hire)
  • Washington Registered Nurse License (within 2 months of hire)
  • Strong leadership skills and ability to coordinate care plans
