About This Home Health RN Position

Responsibilities:
- Assess patient needs and develop individualized care plans in collaboration with the interdisciplinary care team
- Deliver high-quality nursing care to patients in their homes, focusing on prevention, early intervention, and education
- Maintain accurate and detailed records of patient progress and outcomes
Qualifications:
- Degree in Nursing from an accredited institution
- Current Oregon Registered Nurse License
- National Provider BLS - American Heart Association certification within 30 days of hire
- Minimum one year of experience in medical-surgical settings
