Description
Provides education and support services to patients, families and staff. Provides psychosocial assessment, counseling, crisis intervention, referrals and information to patients and families. As a member of the Case Management Department, works collaboratively with the interdisciplinary team to effect positive patient and family outcomes. Serves as resource for social workers and others within the team.

Required Qualifications :
- Master's Degree in Social Work from an accredited graduate school of social work.
- Texas Masters Social Worker License - Texas State Board of Social Worker Examiners upon hire
