Care Team Member - RN Case Manager

The RN Case Manager will work as a care team member to support the needs of the clinics and assume responsibility for the ongoing case management of an assigned panel of patients.

As a case manager, the RN will coordinate with a multidisciplinary team that includes patients, families, physicians, medical assistants, social workers, counselors, and other clinic support staff to provide high quality care.

Key Responsibilities:

• Coordinate patient care with multidisciplinary teams
• Manage assigned panels of patients

Requirements:

• Graduation from an accredited school of nursing
• Alaska Registered Nurse License upon hire
• 2 years of Professional nursing experience
