We are seeking a skilled Licensed Practical Nurse (LPN) to join our home health team, providing high-quality patient care in a dynamic and supportive environment.
Key Responsibilities
- Conduct comprehensive assessments of patients' physical, emotional, and social needs, developing personalized care plans that meet their unique requirements.
- Collaborate with an interdisciplinary team to develop and implement individualized care plans, ensuring seamless coordination of care.
- Monitor patients' progress, adjusting care plans as needed to ensure optimal outcomes.
Requirements
- Graduate of an accredited LPN/LVN school of nursing.
- National Provider BLS - American Heart Association certification within 30 days of hire.
- Oregon Licensed Practical Nurse licensure upon hire.
- Valid driver's license and reliable vehicle for driving to patient homes.
- Good driving record consistent with GOP Safe Driving Policy 330 Incident Point Matrix at time of hire.
Preferred Qualifications
- Experience working in home health, hospice, or public health settings.
