Job Title: Anticoagulation RN Care Coordinator
Description:
The Anticoagulation RN Care Coordinator works collaboratively with referring providers, ACC Pharm-D, front and clinical staff, IHI ACC Clinical Manager and IHI Practice Manager to deliver efficient, compassionate, and high-quality care. This role assesses patients on anticoagulation medication, makes dosage adjustments per protocol based on Point of Care Testing (POCT), or laboratory values.
Responsibilities:
- Assess patients on anticoagulation medication
- Makes dosage adjustments per protocol based on POCT or laboratory values
- Documents in Epic
- Provides detailed ongoing education and in-services to patients, family members, physicians, nurses, and other healthcare professionals regarding drug therapy and anticoagulation management
Requirements:
- Graduate of an accredited RN nursing program
- Montana Registered Nurse License upon hire
- National Provider BLS - American Heart Association upon hire
- 2 years of Clinical RN experience resulting in excellent clinical skills and judgment
Preferred Qualifications:
- Bachelor's Degree in Nursing
- Certification in CACP, CVRN, or Med-Surg upon hire
- Recent Anticoagulation Management, Cardiology, or Med/Surg experience
- Decision support experience, Electronic Medical record (EMR) experience
