Job Summary:
We are seeking a Licensed Practical Nurse (LPN) to join our team in Home Health. As an LPN, you will work alongside an interdisciplinary team to provide whole-person quality care to patients in need.
This is a full-time position with a Monday-Friday schedule from 8:00am-5:00pm. The possible service locations for this role will cover around Humboldt County.
About the Role:

Responsibilities:
- Deliver high-quality patient care while working collaboratively with other healthcare professionals.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Provide education and support to patients and their families to promote health and well-being.
- Maintain accurate and up-to-date records of patient information.
Requirements:
- Currenlty licensed to practice as an LVN in the state of California.
- A minimum of 1 year of experience as a Licensed Vocational Nurse.
- National Provider BLS - American Heart Association certification upon hire.
- Ability to drive and provide proof of driver's license and auto insurance as needed.
Preferred Qualifications:
- 1 year of experience as a Home Health LVN.
