The RN Case Manager will work as a care team member to support the needs of the clinics and assume responsibility for the ongoing case management of an assigned panel of patients. This role requires strong communication and interpersonal skills, as well as the ability to work effectively with a diverse range of stakeholders, including patients, families, physicians, medical assistants, social workers, counselors, and other clinic staff. The ideal candidate will have excellent organizational and time management skills, as well as the ability to prioritize tasks and manage multiple responsibilities simultaneously.

- 2 years of Case Management in a clinic or outpatient setting
