We are seeking a compassionate and skilled RN to join our Home Health Care Services team at Sea Crest Home Health and Hospice in Costa Mesa. Our team is dedicated to providing personalized care to patients with complex medical needs, and we believe that every individual deserves to receive exceptional healthcare in their own home.
Key Responsibilities:
- Assess patients' physical, emotional, and social needs
- Develop and implement personalized care plans to meet patients' unique needs
- Collaborate with other healthcare professionals to ensure seamless care delivery
- Maintain accurate and comprehensive records of patient interactions and progress
Requirements:
- Degree in Nursing from an accredited institution
- Current California Registered Nurse License
- 1 year of professional nursing experience
- BLS certification from the American Heart Association
