Registered Nurses (RNs) work alongside an interdisciplinary team to provide whole-person quality care to patients in need. They are empowered by our shared governance model that includes nursing staff in the process of creating practice standards and expectations.
The Transitional Care Unit is an intermediate and high-acuity department that facilitates recovery from serious injury, illness, disease or medical condition. From the moment patients leave the hospital, we guide and care for their physical, emotional and spiritual needs before they're able to return home, preparing them for the road ahead. Our unit is a comfortable and home-like setting for the patients, where healthcare professionals can handle medical complexities in a more effective way. This includes fractures, complex wound management, IV management, dialysis, oxygen therapy, and Total Parenteral Nutrition (TPN).
