Clinical Licensed Social Worker (LCSW)

Part-time Licensed Clinical Social Worker (LCSW) position available, requiring weekday availability with occasional weekends. Responsibilities include assessing patients' psychosocial status, conducting social evaluations, providing counseling, and maintaining clinical records. The role involves educating patients on advanced directives, serving as a liaison with community agencies, and participating in interdisciplinary care planning. Qualifications include a Master's in social work, current LCSW license, and at least one year of healthcare experience, preferably in hospice.
