Providence Medical Associates is seeking a Nurse Practitioner or Physician Assistant to join their team in Los Angeles, California. This role involves managing complex cases such as heart failure, strokes, dementia, and some wound care, while providing compassionate care planning discussions with patients and families. Candidates with hospice and palliative care experience, as well as expertise in managing senior and complex patients, are highly desired. - Patient visits for high-risk patients (Risk to get re-admitted)
- Goal is to reduce re-admissions
- Experience in the following preferred - Hospice, Palliative, Geriatric, ED, ICU
- Full-time position, Monday-Friday
- Around 6 home visits per day
- Someone that is comfortable discussing care plan with patients and family members
- Compensation is between $165,000 and $190,000 per year
- The posted salary reflects the starting range of total compensation and may include productivity bonuses, incentives related to quality and performance, hospital / ED call stipends, extra shift incentives, and other forms of cash compensation as applicable to the position. In addition, providers typically receive a CME allowance and other benefits offered by their medical group employer. Please note that this salary range is provided in accordance with State law and is subject to variation due to the factors noted above

Providence Medical Associates is a 200-provider multispecialty medical group in Los Angeles serving communities in the greater South Bay, mid-Los Angeles, and San Fernando Valley. PMA provides the full spectrum of integrated care with a strong base of adult primary care physicians, pediatricians and OBGYNs, a broad range of adult specialties, four urgent care centers, a hospitalist team, post-acute ambulatory and SNF care, and pharmacist and behavioral health support.
