Job description
We are seeking board certified Nurse Practitioners FNP, ANP, AGNPs / Physician Assistants in the NYC area to support the statewide NYIA Independent Practitioner Panel. The NP/PA will conduct in-home clinical visits with individuals and write provider orders. Training is virtual classroom and is paid.
Duties
- Responsible for reviewing and making recommendations on consumer assessments for long term care services and supports.
- The incumbent will review the completed Community Health Assessment tool in the NYS Uniform Assessment, in order to assist the Physician in the completion of a Physician’s order.
- Maintain current knowledge of regulatory changes pertaining to quality and validity of assessments.
- Assess cases presented and utilizes clinical competencies to review and validate assessment outcomes.
- Work collaboratively to monitor and address clinical quality and gaps in care
Requirements
- Valid Physician Assistant license in the state of New York
- Must have an active NYS Medicaid ID number
- Must have at least 2 years (3600 hours) of experience in any setting to qualify
- Must be conflict free; Cannot have a Conflict of Interest (COI) - including working for an MLTC, LHCSA, Certified Home Health Agency, Insurance or ISNP
- Laptops are provided to the clinicians
- Ability to work collaboratively within a multidisciplinary team while demonstrating strong communication skills.
- Knowledge of infection control practices is crucial for maintaining patient safety.
